Whole Minds Reports Terms of Service

1) Purpose and Scope
The Add-on helps users efficiently create and format psychoeducational reports in Google Docs. Use must comply with all applicable laws, regulations, and institutional policies.
2) Professional Use & Qualifications (Important)
The Add-on is a formatting/productivity tool only. It does not provide clinical judgment, interpretation, medical/psychological advice, or diagnosis, and it is not a medical device. You are solely responsible for confirming you are licensed/authorized to create, interpret, and use psychoeducational reports in your jurisdiction and setting. Misuse by unqualified users is prohibited.
3) User Responsibilities
- Ensure the accuracy of all text, scores, and information you enter.
- Review each report for accuracy and completeness before use or disclosure.
- Confirm you have all necessary permissions/consents to use any data or images you provide (including student/client data).
- Maintain the security of your Google account and files.
4) Data Storage and Security
Settings and uploaded assets are stored in your Google account via PropertiesService and a dedicated Drive folder. We do not access your stored data. You control sharing and access via Google permissions. Google for Developers
5) Compliance and No Warranty of Compliance
We do not guarantee compliance with FERPA, HIPAA, COPPA, GDPR/UK-GDPR, CPRA, payer, or district requirements. You are responsible for configuring your environment, obtaining any required consents/agreements (e.g., BAAs with Google), and ensuring use is compliant. Google ServicesGoogle Workspace
6) Support & De-identification
Support will never request live student/client data. You must de-identify any materials shared for troubleshooting.
7) Intellectual Property
The Add-on’s code, features, and branding are the property of Whole Minds Reports.
8) Disclaimer of Warranty
The Add-on is provided “AS IS” and “AS AVAILABLE.” We do not warrant uninterrupted or error-free operation or fitness for a particular purpose.
9) Limitation of Liability
To the maximum extent permitted by law, we are not liable for indirect, incidental, special, consequential, or punitive damages. Our aggregate liability for any claim is capped at the greater of US $100 or the fees (if any) you paid for the Add-on in the 12 months before the claim.
10) Indemnification
You agree to indemnify and hold harmless Whole Minds Reports and its developers from claims, damages, liabilities, costs, and expenses arising from: (a) your misuse of the Add-on; (b) your violation of these Terms; (c) your violation of law or third-party rights (including privacy or intellectual-property rights).
11) Termination
We may suspend or terminate access for violations of these Terms or misuse. You may stop using the Add-on at any time. Upon termination, provisions that by nature should survive (e.g., IP, disclaimers, liability limits, indemnity) will survive.
12) Changes to Terms
We may modify these Terms; material changes will be noticed. Continued use after effectiveness constitutes acceptance.
